Each office keeps its own schedule. Call the location you'd like to be seen at and we'll book you on the spot.
You can book a visit with Dr. Moiin directly on Zocdoc and see open times without calling. Zocdoc is convenient, but we release only a limited number of slots there each week — if nothing open works for you, please call the office and we’ll find you something sooner.
New patient? Either path works. Have your insurance card handy, and plan to arrive about 15 minutes early to finish intake paperwork — or fill it out ahead of time with our new patient forms.

If you booked online, our staff reaches out to verify your insurance and let you know what to bring — usually the same business day.
You'll get a reminder before your visit. New patients should arrive about 15 minutes early to complete intake paperwork.
We accept Medicaid along with most major insurance plans. If you're unsure whether we take your plan, call the office before your visit and we'll check it for you — it only takes a minute and saves a surprise later.
